I received the SIM fairly quickly. Shipped in a bubble wrap envelope and the card was in a card-sized ziplock. I had a little heart jump when I couldn't find the Activation code but when I called T-mobile, they didn't ask for it at all (although the t-mobile sim activation website requires it). Was pretty quick to get activated via the phone call (up to 5 minutes via phone representative). I was switching my current account to a new phone/SIM so I can't speak for a fresh new plan activation.
Someone else posted a picture of SIMs taped to a piece of paper but I received an actual card with the PUK code, barcode, & SIM card number (only needed SIM#, personal pin for account, & account owner name via phone). Had an 'activate by' date of 9/21/2019 (2 years from purchase date). They warned that I may lose my Contacts, Voicemails, etc in the transfer and it may take up to 2 hrs to complete but I received the T-mobile text message almost instantaneously and still had all my phone data thereafter.

I Just bought a new Smartphone (Huawei mate 9) and it needed a nano sim card.
When i contacted T-mobile they wanted $25.00 in which i find ridiculous overpriced but thanks to Amazon i found the T-mobile triple cut sim card here at a excellent price!
I Think that is important when writing a review to always important to post the name of the seller so other can benefit from your experience and can be very helpful as you may see more then one seller selling the same product.

It had a expiration date in which i found fantastic "Activate by 07/15/19"
it's the very same card T-mobile sells but at a lower cost.
The activation process was very easy as all i did was to contact T-mobile from my old phone (611) and within minutes i was good to go.
Overall I am very satisfied with my T-mobile 3 in 1 sim card & would definitely recommend both product & seller.

I already had an existing tmobile plan and purchased a new phone which required a micro sim card. I bought this,called tmobile to active the sim and was good to go. If I bought this directly from tmo it would've been $25.
One use for this SIM card is to upgrade from an older style (Standard or Micro) to Nano so it will fit a new model phone. That's what I used it for. I had a good experience similar to other positive reviewers. I think the main thing to understand is that this SIM card is unactivated and does not have an assigned phone number which means the process works the best for transferring an existing account and number.
I popped out the Nano size SIM from the card and installed it in my new phone. I then called T-Mobile support and told them I wanted to transfer my phone number and prepaid account from my old SIM to a new blank unactivated SIM for a new phone. I gave the T-Mobile rep my existing phone number, my account PIN, and the ID number of the new SIM card. After he processed my request, he instructed me to turn the new phone off and on and then wait 2 hours for the change. Surprisingly, it took 10 minutes and the new phone was ready to go.
This was the first time I ever had to do something like this, but I was very happy with the results. First, I was able to get an unlocked Moto phone at a good price and keep my old legacy prepaid plan. Also, I have several options if I ever want to upgrade my account with T-Mobile or any other carrier.
As a last note, I was initially going to cut down my old SIM to Nano size, but decided not to. Cutting may work in some situations, but I didn't want to chance it. This new SIM was only about $6 at the time and I think it was worth it.
Worked perfectly. WARNING: Carefully remove only exact size/shape you require. Do not remove out a smaller size because you will not be able to replace back to larger size. I needed the smallest, nano size so not a problem for me. Had NO problem by telephone getting my Carrier to switch my account from another SIM to this one. Need the very long SIM card number displayed on the new product. .

Tmobile Triple SIM, contains Standard, Micro, Nano SIM
These are as advertised. Triple cut SIM cards that comes attached to SIM card holder. There are indentations to easily extract the desired sized SIM from standard, to micro, to nano. Ordered in July 2017 and the SIM cards expire in November 2019. Once extracted, put in phone and activated on T-Mobile network without issue. However, had to call customer service in order to activate the new SIM card, which is contrary to reported claims that states SIM can be activated online. Nonetheless, it was a simple and quick procedure.
The sim came brand new still in the pop out card. It worked fine. I just had to call T-Mobile customer service to change over to the new SIM. Much cheaper than the $25 taking it to the store. They had to do a few extra things as it was a refurbished phone previously used on their network but they got it all working.
This came as a credit card sized punch out SIM card. All 3 sizes were a choice. Used mini to back up contacts, then punched to Nano size. Called the number on the card and new phone was working in 5 minutes. Did not use website, but PUX number was printed on back of card as well as larger size printing of SIM card number. It did have a blank piece of paper as others have mentioned, so what. Probably to give substance for sending in an envelope. Even came with tool for removing the old card.
I have a regular Post Paid Service (Not prepaid). Worked immediately after hanging up with T-Mobile customer service. Even though I was asked to restart my phone I did not have to. I had a Lumia 830 and just upgraded to Alcatel Idol 4S for windows. In the 830, I had handcut the Micro to Nano and due to the design of the sim card location, the battery was holding it up properly. But for the Alcatel, it has a sim tray, so the sim card has to be of precise size. That is why I had to buy this, because the local T-Mobile store were selling the card for $25. It works as it is supposed to.
Activated 2 of these for T-Mobile postpaid in prep for our international travel and worked fine. lnitialled called TMO to confirm cards were able to be activated and fine. When time came to activate, first CSR said car not valid, but when I pushed furhter, the checked again and both worked fine. We received just card, no packaging but obviously intact in 3-1 card never used or activated.
